New Request for Testing Accommodations
Forms
The previous Form L-15 and Form SA-001 forms
were revised as of April 2005. The new forms were reviewed
and edited by several different groups including: GED Administrators,
GEDTS staff, and GED disability consultants. Throughout this
entire process, the GED Testing Service has worked with the
US Department of Education, the Office of Civil Rights, and
the American Council on Education legal counsel.
The redesigned forms have the necessary information across
four distinct review forms covering specific disabilities.
The correct form to use can be easily identified based on
the candidate's specific disability. There is consistency
of basic information and consistency of definitions for the
documentation requested across all forms. The assessment testing
sections of the forms have also been redesigned so that information
can be easily imputed.
The forms are:

Request for Testing Accommodations-Learning and Other
Cognitive Disabilities;

Request for Testing accommodations-Emotional/Mental Health
Disability;

Request for Testing Accommodations-Attention Deficit/Hyperactivity
Disorder; and
- Request for Testing Accommodations-Physical/Chronic Health
Disability.
